Registered Nurse (RN) – Case Manager
We are seeking a skilled Registered Nurse (RN) – Case Manager to coordinate patient care, support appropriate healthcare services, and promote positive patient outcomes.
Responsibilities:
- Assess patient needs and develop individualized care plans.
- Coordinate care and services across physicians, nurses, specialists, and other healthcare providers.
- Monitor patient progress and ensure appropriate follow-up care.
- Collaborate with the healthcare team to support timely and effective treatment.
- Educate patients and families on care plans, medications, and available resources.
- Assist with discharge planning, transitions of care, and referrals.
- Communicate with insurance providers and other stakeholders as needed.
- Maintain accurate patient records and case management documentation.
Qualifications:
- Graduate of an accredited Nursing program.
- Active Registered Nurse (RN) license as required by state regulations.
- Current BLS certification required.
- CCM or other case management certification preferred.
- Strong knowledge of care coordination, discharge planning, utilization management, and patient advocacy.
- Previous RN case management or care coordination experience preferred.
Pay Rate
$68.09 - $72.97/hour
