RN Case Manager – Non-Surgical Pain
We are seeking a skilled RN Case Manager – Non-Surgical Pain to coordinate patient care and provide clinical support for patients receiving non-surgical pain management services.
Key Responsibilities:
- Assess patient needs and develop individualized care plans.
- Coordinate care between patients, physicians, specialists, and healthcare providers.
- Monitor treatment progress, symptoms, and patient outcomes.
- Educate patients on pain management plans, medications, and self-care.
- Facilitate referrals, follow-up appointments, and appropriate healthcare services.
- Maintain accurate clinical documentation and ensure continuity of care.
Qualifications:
- Graduate of an accredited nursing program.
- Current and active RN license as required by the state.
- Current BLS certification.
- Previous case management, pain management, or outpatient nursing experience preferred.
- Strong clinical assessment, communication, and care-coordination skills.
Pay Rate
$86,000.00 - $117,000.00/year
