RN Case Manager – Hospice
Position Summary
We are seeking an RN Case Manager to coordinate and deliver compassionate hospice care, managing patient care plans, supporting families, and ensuring comfort-focused services for patients at end of life.
Responsibilities
- Assess patients and develop individualized hospice care plans.
- Coordinate interdisciplinary care and manage ongoing patient needs.
- Provide skilled nursing care focused on comfort and symptom management.
- Educate and support patients and families on hospice services and end-of-life care.
- Monitor patient status and adjust care plans as needed.
- Document assessments, interventions, and progress accurately.
- Collaborate with the care team and follow hospice regulations and professional standards.
Qualifications
- Current RN license in good standing.
- Associate or Bachelor’s degree in Nursing.
- Hospice, palliative, or case management experience preferred.
- Strong clinical, communication, and care-coordination skills.
- Compassionate and patient/family-centered approach.
Pay Rate
$71.90 - $76.78/hour
