RN Case Manager – Home Health
Position Summary
Provide skilled nursing care and case management to patients in their homes. Coordinate individualized care plans, monitor patient progress, and collaborate with healthcare providers to support safe recovery and improved patient outcomes.
Responsibilities
- Perform comprehensive patient assessments and develop individualized care plans.
- Coordinate home health services with physicians, therapists, and other healthcare professionals.
- Provide skilled nursing care, including medications, IV therapy, and wound care as prescribed.
- Monitor patient conditions and respond to changes in health status.
- Educate patients and caregivers on medications, treatments, disease management, and home safety.
- Coordinate patient visits, services, and follow-up care.
- Maintain accurate and timely clinical documentation.
- Communicate patient progress and concerns to the healthcare team.
- Follow home health, infection-control, patient safety, and clinical standards.
Qualifications
- Graduate of an accredited nursing program.
- Current and active RN license in the applicable state.
- Previous home health or case management experience preferred.
- Strong assessment, care coordination, communication, and organizational skills.
- Ability to work independently and manage multiple patients effectively.
Pay Rate
$71.90 - $76.78/hour
