Position Summary:

We are seeking a compassionate and organized RN Case Manager – Home Health to coordinate and deliver skilled nursing care for patients in their homes. The RN Case Manager will perform comprehensive assessments, develop individualized care plans, oversee patient progress, and collaborate with interdisciplinary teams to ensure high-quality, patient-centered care.

Key Responsibilities:

  • Conduct comprehensive patient assessments and develop individualized care plans
  • Coordinate and manage home health services in collaboration with physicians and interdisciplinary teams
  • Provide skilled nursing care, including medication management, wound care, and patient education
  • Monitor patient progress and update care plans based on clinical needs and outcomes
  • Coordinate referrals and community resources to support continuity of care
  • Educate patients and caregivers on disease management, medications, and home safety
  • Complete accurate and timely documentation, including OASIS assessments, in the electronic medical record (EMR)
  • Ensure compliance with Medicare guidelines, home health regulations, and agency policies
  • Participate in case conferences, quality improvement initiatives, and care coordination meetings
  • Promote patient independence while ensuring safe, effective care in the home environment

Required Qualifications:

  • Active Registered Nurse (RN) license in the state of practice
  • Graduate of an accredited nursing program (ASN or BSN preferred)
  • Current Basic Life Support (BLS) certification
  • Minimum 1 year of recent home health or case management experience preferred
  • Experience completing OASIS assessments and home health documentation preferred
  • Strong clinical assessment, care coordination, and critical-thinking skills
  • Excellent communication, organizational, and patient education abilities
  • Valid driver's license and reliable transportation, if required for patient visits

Pay Rate
$71.90 - $76.78/hour
