RN, Care Coordinator
Position Summary
We are seeking a Registered Nurse (RN) Care Coordinator to coordinate patient care and support patients throughout their healthcare journey. The RN will assess patient needs, coordinate services, communicate with healthcare providers, and help ensure safe and effective care.
Responsibilities
- Assess patient needs and develop appropriate care plans.
- Coordinate care and services across healthcare providers and settings.
- Communicate with patients, families, physicians, and care teams.
- Monitor patient progress and follow up on care needs.
- Assist with referrals, appointments, and healthcare resources.
- Educate patients and families on treatment plans and available services.
- Maintain accurate and timely patient records.
- Identify barriers to care and help develop solutions.
- Promote quality, safety, and continuity of care.
Qualifications
- Active Registered Nurse (RN) license.
- Nursing degree from an accredited program.
- Experience in care coordination, case management, or a healthcare setting preferred.
- Strong communication, organization, and clinical assessment skills.
- Ability to coordinate effectively with patients and multidisciplinary healthcare teams.
Pay Rate
$71.90 - $76.78/hour
