Job Title: Clinical Pharmacist
Position Summary
The Clinical Pharmacist is responsible for providing expert pharmaceutical care to ensure the safe and effective use of medications. This role includes medication dispensing, patient counseling, medication therapy management, and collaboration with healthcare professionals to optimize patient outcomes and support high-quality patient care.
Responsibilities
- Accurately dispense medications and provide patient counseling regarding medication use, side effects, and drug interactions
- Collaborate with physicians, nurses, and healthcare providers to develop and optimize medication therapy plans
- Conduct Medication Therapy Management (MTM) reviews to identify and resolve medication-related concerns
- Maintain accurate patient records and ensure compliance with pharmacy regulations and standards
- Monitor pharmacy inventory, manage medication stock levels, and oversee expiration tracking
- Educate patients on disease prevention, medication adherence, wellness, and healthy lifestyle practices
- Participate in community outreach and health promotion activities
- Support quality improvement initiatives and pharmacy best practices
- Maintain knowledge of current pharmacotherapy guidelines, medications, and regulatory updates
- Participate in ongoing professional development and continuing education opportunities
Qualifications
- Doctor of Pharmacy (Pharm.D.) degree from an accredited institution required
- Active Pharmacist license in the applicable state required
- Strong knowledge of pharmacology, medication therapy, and therapeutic protocols
- Previous retail or clinical pharmacy experience preferred
- Excellent communication, interpersonal, and patient counseling skills
- Proficiency with pharmacy management systems and healthcare technology preferred
- Ability to work efficiently in a fast-paced environment and manage multiple priorities
