Rehabilitation Physical Therapist (PT) – Long-Term Care (LTC)
We are seeking a skilled Rehabilitation Physical Therapist (PT) to provide individualized therapy to long-term care residents, helping improve mobility, strength, function, and independence.
Key Responsibilities:
- Evaluate residents and develop individualized treatment plans.
- Provide therapeutic exercises, gait, balance, and mobility training.
- Monitor progress and modify treatment plans as needed.
- Educate residents and caregivers on exercises and safety techniques.
- Collaborate with nursing staff, physicians, and the rehabilitation team.
- Maintain accurate treatment records and documentation.
Qualifications:
- Graduate of an accredited Physical Therapy program.
- Active Physical Therapist (PT) license as required by state regulations.
- Current BLS certification preferred/required.
- Strong knowledge of geriatric rehabilitation and therapeutic techniques.
- Previous LTC/SNF or rehabilitation experience preferred.
Pay Rate
$52.85 - $57.72/hour
