Position Summary:

We are seeking a compassionate and skilled Rehabilitation Physical Therapist – Acute/Inpatient to provide comprehensive physical therapy services to hospitalized patients recovering from illness, injury, surgery, or other acute medical conditions. The Physical Therapist will evaluate patients, develop individualized treatment plans, and collaborate with the interdisciplinary healthcare team to restore mobility, improve functional independence, and support safe discharge planning.


Key Responsibilities:

  • Perform comprehensive physical therapy evaluations to assess mobility, strength, balance, coordination, endurance, range of motion, and functional abilities
  • Develop, implement, and modify individualized treatment plans based on patient diagnoses, goals, and clinical progress
  • Provide therapeutic interventions including therapeutic exercises, gait training, transfer training, balance training, neuromuscular re-education, manual therapy, and functional mobility training
  • Evaluate and treat patients with orthopedic, neurological, cardiopulmonary, post-surgical, trauma, stroke, spinal cord injury, and medically complex conditions
  • Monitor patient progress and adjust treatment plans to optimize recovery and functional outcomes
  • Educate patients and caregivers on home exercise programs, mobility techniques, fall prevention, body mechanics, and safe use of assistive devices
  • Recommend and train patients in the use of walkers, canes, crutches, wheelchairs, orthotics, prosthetics, and other adaptive equipment
  • Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, case managers, and other healthcare professionals to coordinate patient care and discharge planning
  • Maintain accurate and timely documentation of evaluations, treatment sessions, progress notes, and discharge summaries in Electronic Medical Records (EMR)
  • Participate in interdisciplinary rounds, quality improvement initiatives, and patient care conferences
  • Ensure compliance with infection prevention protocols, patient safety standards, ethical guidelines, and regulatory requirements
  • Promote patient independence, functional recovery, and safe transition to the next level of care

Required Qualifications:

  • Active, unrestricted Physical Therapist (PT) license in the state of practice
  • Doctor of Physical Therapy (DPT) degree from an accredited Physical Therapy program
  • Current BLS/CPR certification required
  • Minimum 1 year of recent acute care or inpatient rehabilitation physical therapy experience preferred
  • Strong knowledge of acute care rehabilitation, mobility training, therapeutic exercise, and evidence-based physical therapy practices
  • Excellent clinical assessment, documentation, critical-thinking, communication, and interpersonal skills
  • Ability to work independently and collaboratively within a multidisciplinary healthcare team
  • Experience with Electronic Medical Record (EMR) systems preferred
  • Commitment to delivering compassionate, patient-centered rehabilitation care
