Registered Nurse – Case Manager
We are seeking a compassionate and organized Registered Nurse Case Manager to coordinate patient care and ensure effective utilization of healthcare resources. The RN Case Manager will collaborate with patients, families, and interdisciplinary teams to develop care plans, facilitate transitions of care, and promote positive patient outcomes.
Responsibilities:
• Assess patient needs and develop individualized care plans.
• Coordinate care and services across the healthcare continuum.
• Collaborate with physicians, nurses, and other healthcare professionals to support patient outcomes.
• Facilitate discharge planning and transitions of care.
• Educate patients and families regarding treatment plans and available resources.
• Maintain accurate and timely documentation of case management activities.
Qualifications:
• Active and unrestricted Registered Nurse (RN) license.
• Graduation from an accredited nursing program.
• Strong knowledge of case management principles and care coordination.
• Excellent communication, organizational, and problem-solving skills.
• Ability to work effectively with interdisciplinary teams.
• Previous case management, utilization review, or acute care experience preferred.
Pay Rate
$63.79 - $68.67/hour
