RN Case Manager
We are seeking a dedicated and compassionate RN Case Manager to join our healthcare team. The RN Case Manager will be responsible for coordinating patient care, developing care plans, and ensuring patients receive appropriate medical services while maintaining high standards of quality care.
Responsibilities:
- Ability to assess, evaluate and drive discharge plan.
- Coordinate services to the patient's appropriate level of care.
- Ensure that treatment plan is initiated, evaluated and re-evaluated daily.
- Follow the progress and lack thereof daily.
- Monitor length of stay and identify barriers to discharge.
Qualifications:•
- Active and unrestricted Registered Nurse (RN) license in the state of practice
- Minimum 3 years of nursing experience; case management experience preferred
- Strong leadership, communication, and organizational skills
- Ability to coordinate patient care and collaborate with interdisciplinary teams
- Experience in care planning, discharge planning, and utilization review preferred
- Knowledge of healthcare regulations, compliance, and documentation standards
- Previous leadership or nursing management experience preferred
- BLS Certification required
Pay Rate
$49.44 - $54.32/hour
