Position Summary

An RN Case Manager is responsible for coordinating and managing patient care throughout the healthcare continuum. They collaborate with physicians, nurses, therapists, patients, and families to develop care plans, facilitate safe discharges, ensure appropriate utilization of healthcare resources, and improve patient outcomes.

Key Responsibilities

  • Assess patients' medical, psychosocial, and discharge planning needs.
  • Develop, implement, and monitor individualized care plans.
  • Coordinate care with physicians, nursing staff, therapists, social workers, and other healthcare professionals.
  • Facilitate discharge planning and arrange post-acute care services.
  • Ensure appropriate utilization of healthcare resources and services.
  • Review patient progress and modify care plans as needed.
  • Educate patients and families regarding treatment plans and available community resources.
  • Maintain accurate and timely documentation in the electronic medical record (EMR).

Required Skills

  • Strong clinical assessment and critical thinking skills.
  • Care coordination and discharge planning experience.
  • Knowledge of utilization review and case management principles.
  • Proficiency with EMR systems.
  • Knowledge of Medicare, Medicaid, commercial insurance, and regulatory guidelines.

Basic Requirements

  • Active RN license (state-specific or compact license, if applicable).
  • Associate Degree in Nursing (ADN) required; BSN preferred by many facilities.

Pay Rate
$49.44 - $54.32/hour
