Registered Nurse (RN) – Case Manager

Job Summary

We are seeking a compassionate and experienced Registered Nurse (RN) Case Manager to coordinate and manage patient care throughout the healthcare continuum. The ideal candidate will collaborate with physicians, patients, families, and interdisciplinary teams to develop individualized care plans, ensure appropriate resource utilization, and promote positive patient outcomes.

Responsibilities

  • Assess patients' medical, psychosocial, and discharge planning needs.
  • Develop, implement, and monitor individualized care plans.
  • Coordinate patient care with physicians, nurses, therapists, social workers, and other healthcare professionals.
  • Facilitate safe and timely discharge planning and care transitions.
  • Review medical records to ensure appropriate level of care and resource utilization.
  • Educate patients and families regarding treatment plans, medications, and community resources.
  • Advocate for patients to ensure quality, cost-effective care.
  • Maintain accurate and timely documentation in the electronic medical record (EMR).
  • Ensure compliance with facility policies and regulatory requirements.

Qualifications

  • Active Registered Nurse (RN) license.
  • Current AHA BLS Certification (Required).
  • Minimum 1–2 years of recent Case Management experience (preferred).
  • Previous acute care or hospital nursing experience preferred.
  • Strong assessment, communication, organizational, and critical thinking skills.
  • Experience with discharge planning, utilization review, or care coordination is a plus.
  • Proficiency with electronic medical records (EMR).

Pay Rate
$63.79 - $68.67/hour
