Position Summary:

We are seeking a qualified and compassionate Registered Nurse (RN) to provide skilled home health nursing services. The RN will be responsible for patient admissions, reassessments, re-certifications, and discharges in accordance with physician orders. This role involves comprehensive patient assessments, care coordination, supervision of clinical support staff, and accurate clinical documentation to ensure safe, compliant, and effective patient care in the home setting.


Key Responsibilities:

• Conduct initial home health admissions, reassessments, re-certifications, and discharges
• Perform comprehensive patient assessments and ongoing evaluations
• Initiate and implement Plans of Care in accordance with physician orders
• Monitor patient conditions, symptoms, responses, and progress
• Observe, assess, and document clinical findings accurately and timely
• Educate patients and caregivers on disease processes, medications, and individualized care plans
• Provide education on safe in-home care techniques
• Coordinate patient services to ensure continuity of care
• Supervise and oversee care provided by LPNs and Home Health Aides (HHAs)
• Communicate changes in patient condition to physicians and interdisciplinary team members
• Participate in case conferences to support coordinated patient care
• Maintain accurate and up-to-date clinical records per agency policy
• Adhere to state regulations, agency policies, and professional nursing standards
• Participate in ongoing education and skill development activities


Required Qualifications:

• Active, unrestricted Registered Nurse (RN) license in the state of practice
• Associate’s or Bachelor’s degree in Nursing
• Minimum 1 year of recent nursing experience required; home health experience preferred
• Strong patient assessment, documentation, and care coordination skills
• Experience supervising LPNs and HHAs preferred
• Knowledge of wound care and home health nursing practices preferred
• Excellent communication, organization, and time-management skills
• Ability to work independently in a home-based care environment

Pay Rate
$45.00 - $56.74/hour
