Position Summary:
We are seeking an organized and compassionate Registered Nurse to provide home health case management. The RN will complete patient admissions, perform clinical assessments, develop personalized Plans of Care, supervise caregivers, and ensure safe, structured, and patient-focused nursing support in home and assisted living environments.
Key Responsibilities:
• Conduct patient admissions and clinical assessments in home settings.
• Build and update individualized care plans based on patient conditions and needs.
• Oversee medication management and monitor changes in patient status.
• Supervise home health aides and LPNs to ensure safe and efficient patient care.
• Provide training and education to caregivers and care teams on proper care execution.
• Document clinical evaluations and interventions accurately and timely.
• Coordinate patient support services through interdisciplinary care collaboration.
Required Qualifications:
• Active Registered Nurse (RN) license in Connecticut or compact eligibility.
• Associate’s Degree in Nursing (Bachelor’s preferred).
• 1–3 years of clinical or home-health experience accepted; case management preferred.
• Bilingual English/Russian is a strong advantage.
• Excellent communication, organization, patient assessment, and multitasking skills.

Pay Rate
$40.00 - $40.00/hour
