Position Summary:
We are seeking a compassionate and experienced Home Health Care Registered Nurse to provide skilled nursing services to patients in their homes. The RN will assess patient needs, develop individualized care plans, provide treatments, educate patients and caregivers, and coordinate care with the healthcare team.
Key Responsibilities:
- Conduct comprehensive nursing assessments during home visits.
- Develop, implement, and evaluate individualized patient care plans.
- Provide skilled nursing care based on patient needs and physician orders.
- Administer medications, injections, IV therapies, and prescribed treatments as appropriate.
- Provide wound care, catheter care, ostomy care, and other skilled nursing interventions.
- Monitor patient conditions and report significant changes to physicians or the care team.
- Educate patients and caregivers on medications, disease management, treatment plans, and home safety.
- Coordinate care with physicians, therapists, social workers, home health aides, and other healthcare professionals.
- Participate in care planning, case conferences, and discharge planning.
- Maintain accurate and timely clinical documentation in the EMR.
- Promote patient independence, safety, and effective management of health conditions at home.
- Follow infection prevention, HIPAA, patient safety, and home health policies.
Required Qualifications:
- Active, unrestricted Registered Nurse (RN) license in the state of practice.
- ADN or BSN from an accredited nursing program.
- Current BLS certification required.
- Minimum 1 year of recent home health or clinical nursing experience preferred.
- Strong knowledge of patient assessment, medication administration, wound care, and chronic disease management.
- Excellent clinical judgment, communication, organization, and time-management skills.
- Ability to work independently and manage multiple home visits.
- Valid driver's license and reliable transportation for patient visits.
- Experience with EMR systems preferred.
Preferred Qualifications:
- BSN preferred.
- Previous home health or Medicare-certified home care experience.
- OASIS experience preferred.
- Experience caring for geriatric, post-surgical, medically complex, or chronically ill patients.
- Wound care, IV therapy, or infusion experience preferred.
- Strong patient and caregiver education skills.
- Commitment to compassionate, patient-centered care.
Pay Rate
$56.66 - $61.53/hour
