Position Summary:
We are seeking a compassionate Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The RN will assess patient needs, develop care plans, provide treatments, educate patients and caregivers, and coordinate care with physicians and other healthcare professionals.
Key Responsibilities:
- Conduct comprehensive in-home patient assessments and develop individualized care plans.
- Provide skilled nursing services including medication administration, wound care, IV therapy, and other treatments as ordered.
- Monitor patient health status and identify changes requiring medical intervention.
- Educate patients and caregivers on medications, disease management, safety, and home care techniques.
- Perform wound, catheter, ostomy, and other specialized nursing care as required.
- Coordinate care with physicians, therapists, social workers, and other members of the healthcare team.
- Maintain accurate and timely clinical documentation in the EMR.
- Communicate patient progress and changes in condition to the appropriate healthcare providers.
- Support patients in achieving health goals and maintaining independence at home.
- Follow infection prevention, HIPAA, patient safety, and home health policies.
- Participate in care coordination, quality improvement, and continuing education activities.
Required Qualifications:
- Active, unrestricted Registered Nurse (RN) license in the state of practice.
- ADN or BSN from an accredited nursing program.
- Current BLS certification required.
- Minimum 1 year of recent home health or relevant clinical nursing experience preferred.
- Strong knowledge of patient assessment, wound care, medication administration, and care planning.
- Excellent communication, organization, clinical judgment, and time-management skills.
- Valid driver's license and reliable transportation as required for home visits.
- Experience with Home Health EMR systems preferred.
Preferred Qualifications:
- Previous home health nursing experience preferred.
- Experience with OASIS assessments and home health documentation preferred.
- Experience with wound care, IV therapy, ostomy care, and chronic disease management.
- Strong ability to work independently and manage multiple patient visits.
- Commitment to compassionate, patient-centered care.
Pay Rate
$56.66 - $61.53/hour
