Physical Therapist Assistant (PTA) – Home Health
We are seeking a compassionate and motivated Physical Therapist Assistant to provide high-quality rehabilitative care to patients in home health settings. The PTA will deliver therapy services under the supervision of a licensed Physical Therapist, assist patients in improving mobility and functional independence, and support individualized treatment plans to promote safe recovery in the home environment.
Responsibilities:
• Provide physical therapy treatments to patients in their homes as outlined in the established plan of care
• Assist patients with therapeutic exercises, gait training, balance activities, and functional mobility tasks
• Monitor patient progress and report changes in condition to the supervising Physical Therapist
• Educate patients and caregivers on home exercise programs, safety techniques, and fall prevention
• Document all therapy services, patient responses, and progress accurately and timely
• Assist with the use of adaptive equipment and mobility aids as needed
• Maintain compliance with home health regulations, safety standards, and infection control practices
• Communicate effectively with interdisciplinary care team members to support coordinated patient care
• Support patient independence while ensuring safety within the home environment
Qualifications:
• Graduate of an accredited Physical Therapist Assistant program
• Active and unrestricted state licensure as a Physical Therapist Assistant
• Experience in home health, outpatient, or rehabilitation settings preferred
• Strong knowledge of therapeutic exercises, mobility training, and rehabilitation techniques
• Excellent communication, organization, and patient-care skills
• Ability to work independently and manage a flexible schedule in home-based settings
• Valid driver’s license and ability to travel between patient homes as required
• Commitment to providing compassionate, patient-centered rehabilitative care
