Position Summary
We are seeking a Physical Therapist (PT) to evaluate and treat patients with physical and functional limitations. The PT will develop individualized treatment plans, provide therapeutic interventions, monitor patient progress, and collaborate with the healthcare team to support safe and effective patient care.
Key Responsibilities
- Evaluate patients' mobility, strength, balance, coordination, and functional abilities.
- Develop individualized treatment plans based on patient needs and clinical findings.
- Provide therapeutic exercises, manual therapy, mobility training, and other appropriate interventions.
- Monitor patient progress and modify treatment plans as needed.
- Educate patients and caregivers on treatment plans, exercises, and home programs.
- Maintain accurate and timely clinical documentation.
- Communicate patient progress and concerns with physicians and other healthcare professionals.
- Promote patient safety during therapy sessions and mobility activities.
- Follow established clinical, safety, infection prevention, and documentation standards.
- Participate in quality improvement activities.
- Perform other related duties as assigned.
Required Qualifications
- Doctor of Physical Therapy (DPT) or equivalent accredited degree.
- Current, active, and unrestricted Physical Therapist license in the applicable state.
- Strong knowledge of physical therapy principles, anatomy, and rehabilitation techniques.
- Excellent clinical assessment, communication, and interpersonal skills.
- Ability to develop and implement individualized treatment plans.
- Ability to work independently and as part of a multidisciplinary team.
Preferred Qualifications
- Previous physical therapy experience in the assigned clinical setting.
- Experience treating diverse patient populations.
- Familiarity with electronic medical records (EMR).
- Strong patient education and care coordination skills.
- BLS certification preferred/required based on assignment requirements.
Pay Rate
$64.28 - $69.16/hour
