Position Summary:

We are seeking a skilled Physical Therapist to evaluate and treat patients with physical and functional limitations. The PT will develop individualized treatment plans, provide therapeutic interventions, monitor progress, and help patients improve mobility, strength, independence, and quality of life.

Key Responsibilities:

  • Perform comprehensive physical therapy evaluations and functional assessments.
  • Assess strength, range of motion, balance, gait, mobility, and functional limitations.
  • Develop and implement individualized treatment plans based on patient needs and goals.
  • Provide therapeutic exercises, manual therapy, gait training, balance activities, and functional mobility interventions.
  • Assist patients with transfers, ambulation, and appropriate assistive devices.
  • Monitor patient progress and modify treatment plans as clinically appropriate.
  • Educate patients and caregivers on exercises, mobility, safety, and home programs.
  • Recommend appropriate assistive equipment and rehabilitation strategies.
  • Collaborate with physicians, nurses, therapists, and other healthcare professionals.
  • Participate in care planning, discharge planning, and interdisciplinary communication.
  • Maintain accurate and timely evaluations, treatment notes, progress reports, and discharge documentation.
  • Follow infection prevention, HIPAA, patient safety, and organizational policies.

Required Qualifications:

  • Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program.
  • Active Physical Therapist license in the state of practice.
  • Current BLS certification required.
  • Minimum 1 year of recent Physical Therapy experience preferred.
  • Strong knowledge of rehabilitation, therapeutic exercise, functional mobility, and patient safety.
  • Excellent clinical judgment, communication, organization, and time-management skills.
  • Ability to work effectively with patients and multidisciplinary healthcare teams.
  • Experience with EMR systems preferred.

Preferred Qualifications:

  • Experience in acute care, outpatient, inpatient rehabilitation, skilled nursing, or home health settings.
  • Experience treating orthopedic, neurological, geriatric, post-surgical, or medically complex patients.
  • Strong knowledge of gait training, balance rehabilitation, manual therapy, and therapeutic exercise.
  • Experience with patient and caregiver education.
  • Strong care coordination and discharge planning skills.
  • Commitment to compassionate, patient-centered care.

Pay Rate
$60.47 - $65.35/hour
