Position Summary:
We are seeking a compassionate and skilled Physical Therapist (PT) to evaluate and treat patients with injuries, illnesses, disabilities, and movement disorders. The Physical Therapist will develop individualized treatment plans, provide evidence-based rehabilitation services, and collaborate with interdisciplinary healthcare teams to restore mobility, reduce pain, improve functional independence, and enhance patients' overall quality of life.
Key Responsibilities:
- Perform comprehensive physical therapy evaluations to assess strength, mobility, balance, coordination, range of motion, posture, endurance, and functional abilities
- Develop, implement, and modify individualized treatment plans based on patient needs and rehabilitation goals
- Provide therapeutic interventions including therapeutic exercises, gait training, balance training, neuromuscular re-education, manual therapy, and functional mobility training
- Evaluate and treat patients with orthopedic, neurological, musculoskeletal, cardiopulmonary, post-surgical, and medically complex conditions
- Monitor patient progress and adjust treatment plans to maximize functional outcomes
- Educate patients and caregivers on home exercise programs, body mechanics, injury prevention, pain management, fall prevention, and safe use of assistive devices
- Recommend and train patients in the use of walkers, canes, crutches, wheelchairs, orthotics, prosthetics, and other mobility aids
- Collaborate with physicians, occupational therapists, speech-language pathologists, nurses, case managers, and other healthcare professionals to coordinate patient care
- Maintain accurate and timely documentation in Electronic Medical Records (EMR)
- Participate in interdisciplinary care planning, discharge planning, and quality improvement initiatives
- Ensure compliance with infection prevention protocols, patient safety standards, HIPAA regulations, and facility policies
- Promote patient independence, functional recovery, and overall wellness through compassionate, patient-centered care
Required Qualifications:
- Active, unrestricted Physical Therapist (PT) license in the state of practice
- Doctor of Physical Therapy (DPT) degree from an accredited Physical Therapy program
- Current BLS/CPR certification required
- Minimum 1 year of recent Physical Therapist experience preferred
- Strong knowledge of physical therapy principles, rehabilitation techniques, anatomy, physiology, biomechanics, and evidence-based practice
- Excellent clinical assessment, communication, documentation, critical-thinking, and organizational skills
- Ability to work independently and collaboratively within a multidisciplinary healthcare team
- Experience with Electronic Medical Record (EMR) systems preferred
- Commitment to delivering compassionate, patient-centered rehabilitation care
