Home Care HCH Mobile Units MV - PT
Department: PT- home health PT
Common diagnosis / Types of patients/patient demographics multidisciplinary cases, adults: 78% Medicare patients: elderly, rehab, CVA, inpatient surgical referrals: ortho, surgical, wounds (no lymphedema)
Additional Job Duties involved? "In home therapy, Admissions, existing patients, recertifications (few Assisted Living patients)
Limited oxygen requirements
no mileage re-imbursement and up to 60 miles east and west"
Experience or 'Must have' skills & common job duties "Admissions, existing, recertifications.
MUST have some home health experience, prefer post grad home health
Must have 1 year post grad PT experience
First time travelers okay
Must have Ortho experience "
How many patients do they see per day? 4-6, can start shift from home do not need to stop in an office
50 mile radius both east and west, about 60 miles north up to Sun Valley and about 40 miles south to the ID/NV boarder.
Other Certs Required for all PT: BLS AHA
Interdisciplinary: Monday and Wednesdays multidisciplinary meetings to discuss current admissions
EMR Epic with remote client offshoot
PT Manager/Supervisor Supervisors available, Kelli, and Manager Bryan
Rehab Aides: PTA's none for home health
Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?) M-F 830a-5p
Weekend Requirements none for home health
Holiday Requirements none for home health
Scrub Color/Dress Code Business Casual and any color scrubs
Department specific orientation Epic classes shadow up to 1 week with another therapist
Pay Rate
$64.47 - $69.35/hour
