Position Summary:
We are seeking a compassionate and skilled Physical Therapist (PT) – Nursing & Rehabilitation to provide comprehensive rehabilitation services to patients recovering from illness, injury, surgery, or chronic medical conditions. The Physical Therapist will evaluate patients, develop individualized treatment plans, and implement therapeutic interventions to improve mobility, strength, balance, functional independence, and overall quality of life.
Key Responsibilities:
- Evaluate patients' physical function, mobility, strength, balance, coordination, and rehabilitation needs
- Develop and implement individualized physical therapy treatment plans
- Provide therapeutic exercises, gait training, balance training, transfer training, manual therapy, and functional mobility interventions
- Assess and recommend assistive devices, adaptive equipment, and mobility aids
- Monitor patient progress and modify treatment plans based on clinical outcomes
- Educate patients, families, and caregivers on home exercise programs, fall prevention, body mechanics, and safe mobility techniques
- Complete accurate evaluations, treatment notes, progress reports, and discharge documentation in the Electronic Medical Record (EMR)
- Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, case managers, and other healthcare professionals
- Participate in interdisciplinary care conferences and discharge planning
- Ensure compliance with infection prevention protocols, HIPAA regulations, patient safety standards, and facility policies
- Participate in quality improvement initiatives and continuing education activities
Required Qualifications:
- Doctor of Physical Therapy (DPT) or Master's degree in Physical Therapy from an accredited program
- Active, unrestricted Physical Therapist (PT) license in the state of practice
- Current BLS certification required
- Minimum 1 year of recent physical therapy experience in a rehabilitation, skilled nursing, or post-acute care setting preferred
- Strong knowledge of orthopedic, neurological, geriatric, and post-surgical rehabilitation principles
- Excellent communication, critical-thinking, organizational, and interpersonal skills
- Experience with Electronic Medical Record (EMR) systems preferred
Preferred Qualifications:
- Experience in skilled nursing facilities (SNF), inpatient rehabilitation, long-term care, transitional care, or acute rehabilitation settings
- Previous travel or contract therapy experience preferred
- Experience treating orthopedic, neurological, geriatric, and post-surgical patient populations
- Board Certification (OCS, GCS, Company, or other ABPTS specialty certification) preferred
- Strong teamwork, adaptability, and patient-centered care skills
Pay Rate
$58.94 - $63.82/hour
