Physical Therapist (PT) – Long-Term Care (LTC)
We are seeking a compassionate Physical Therapist to provide rehabilitative care to residents in a long-term care setting. The PT will evaluate residents, develop individualized treatment plans, and help improve mobility, strength, balance, and functional independence.
Responsibilities:
- Evaluate residents and develop individualized treatment plans.
- Provide therapeutic exercises, manual therapy, and mobility training.
- Monitor resident progress and modify treatment plans as needed.
- Educate residents, families, and caregivers on therapy techniques and home exercise programs.
- Maintain accurate documentation of evaluations, treatments, and progress.
- Collaborate with the interdisciplinary team to support resident care.
Qualifications:
- Active and unrestricted Physical Therapist (PT) license.
- Graduation from an accredited Physical Therapy program.
- BLS certification required.
- Strong knowledge of rehabilitation techniques, mobility training, and patient care.
- Previous Long-Term Care, Skilled Nursing, or Rehabilitation experience preferred.
Pay Rate
$51.32 - $56.20/hour
