Physical Therapist (PT) – Home Health
We are seeking a skilled Home Health Physical Therapist (PT) to provide individualized rehabilitation services to patients in their homes, helping improve mobility, strength, function, and independence.
Key Responsibilities:
- Evaluate patients in their home environment and develop individualized treatment plans.
- Provide therapeutic exercises, gait, balance, and mobility training.
- Assess home safety and recommend appropriate modifications or assistive devices.
- Monitor patient progress and adjust treatment plans as needed.
- Educate patients and caregivers on home exercises, mobility, and safety.
- Coordinate care with physicians and other healthcare professionals.
- Maintain accurate clinical documentation and visit records.
Qualifications:
- Graduate of an accredited Physical Therapy program.
- Active PT license as required by state regulations.
- Current BLS certification preferred/required.
- Valid driver’s license and reliable transportation may be required.
- Previous Home Health or community-based rehabilitation experience preferred.
Pay Rate
$60.47 - $65.35/hour
