PT – Home Health
Provide skilled physical therapy services to patients in their homes to improve mobility, strength, balance, and independence. The Home Health Physical Therapist will evaluate patients, develop individualized treatment plans, provide therapy, and educate patients and caregivers on safe mobility.
Responsibilities
- Perform comprehensive physical therapy evaluations in the home.
- Develop and implement individualized treatment plans.
- Provide therapeutic exercises, gait training, balance training, and mobility interventions.
- Assess functional limitations and monitor patient progress.
- Educate patients and caregivers on home exercises, mobility, and safety.
- Recommend appropriate assistive devices and adaptive techniques.
- Coordinate care with physicians, nurses, and other healthcare professionals.
- Document evaluations, treatments, and patient progress accurately.
- Adjust treatment plans based on patient response and clinical needs.
Qualifications
- Current Physical Therapist (PT) license.
- Graduate of an accredited Physical Therapy program.
- Previous home health experience preferred.
- Strong clinical assessment and treatment-planning skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and travel between patient homes.
Pay Rate
$56.66 - $61.53/hour
