Position Summary:
We are seeking a compassionate and skilled Physical Therapist (PT) – Home Health to provide rehabilitative care to patients in their homes. The PT will evaluate functional abilities, develop individualized treatment plans, provide therapeutic interventions, and educate patients and caregivers to improve mobility, independence, and quality of life.
Key Responsibilities:
- Conduct comprehensive physical and functional assessments in patients' homes
- Develop and implement individualized treatment plans based on patient needs and goals
- Provide therapeutic exercises, gait training, balance activities, and mobility interventions
- Educate patients and caregivers on home exercise programs, safety, and injury prevention
- Monitor patient progress and modify treatment plans as needed
- Recommend appropriate assistive devices and home modifications to improve safety and independence
- Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate care
- Maintain accurate and timely documentation, including required home health assessments
- Ensure compliance with Medicare guidelines, home health regulations, and clinical standards
- Promote patient independence and safe functional recovery in the home environment
Required Qualifications:
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
- Active Physical Therapist (PT) license in the state of practice
- Current Basic Life Support (BLS) certification preferred
- Minimum 1 year of recent physical therapy experience preferred
- Home health experience preferred
- Strong clinical assessment, communication, and care coordination skills
- Ability to work independently and manage a home-based patient caseload
- Valid driver's license and reliable transportation for patient visits
Pay Rate
$60.47 - $65.35/hour
