Physical Therapist – Home Health (PT)
We are seeking a compassionate and independent Physical Therapist to provide rehabilitative care to patients in their homes. The Home Health PT will evaluate patient needs, develop individualized treatment plans, and deliver therapy to improve mobility, strength, functional independence, and overall quality of life.
Responsibilities:
• Evaluate patients in their home environment and develop individualized treatment plans.
• Provide therapeutic exercises, manual therapy, and mobility training.
• Assess patient progress and modify treatment plans as needed.
• Educate patients and caregivers on home exercise programs, safety, and injury prevention.
• Collaborate with physicians and the interdisciplinary healthcare team to coordinate care.
• Maintain accurate and timely documentation of patient evaluations, treatments, and progress.
• Promote patient independence and functional recovery in the home setting.
Qualifications:
• Active and unrestricted Physical Therapist (PT) license.
• Graduation from an accredited Physical Therapy program.
• BLS certification required.
• Strong assessment, communication, and patient care skills.
• Ability to work independently and manage a flexible caseload.
• Previous home health or rehabilitation experience preferred.
Pay Rate
$56.66 - $61.53/hour
