Primary Registered Nurse (RN) – Community Hospice
We are seeking a compassionate Primary RN – Community Hospice to provide patient-centered hospice care in the community, focusing on comfort, symptom management, and quality of life.
Key Responsibilities:
- Assess patients and develop individualized hospice care plans.
- Manage pain, symptoms, medications, and comfort needs.
- Coordinate care with physicians, caregivers, and the hospice interdisciplinary team.
- Educate patients and families on disease progression and end-of-life care.
- Monitor patient status and update care plans as needed.
- Maintain accurate clinical documentation and hospice records.
Qualifications:
- Graduate of an accredited Nursing program.
- Active RN license as required by state regulations.
- Current BLS certification required.
- Previous hospice, palliative care, home health, or oncology experience preferred.
- Strong communication, assessment, and compassionate patient-care skills.
Pay Rate
$49.03 - $53.91/hour
