Position Summary:
We are seeking a compassionate and skilled Physical Therapist (PT) to evaluate, develop, and implement individualized treatment plans for patients recovering from injury, surgery, illness, or physical disabilities. The Physical Therapist will help patients improve mobility, reduce pain, restore function, and maximize independence while collaborating with the interdisciplinary healthcare team to achieve optimal patient outcomes.
Key Responsibilities:
- Perform comprehensive physical therapy evaluations and functional assessments.
- Develop and implement individualized treatment plans based on physician referrals and patient goals.
- Provide therapeutic exercises, manual therapy, gait training, balance training, mobility training, and other rehabilitation interventions.
- Assess and recommend assistive devices, orthotics, adaptive equipment, and mobility aids.
- Monitor patient progress and modify treatment plans as needed to achieve rehabilitation goals.
- Educate patients, families, and caregivers on home exercise programs, injury prevention, body mechanics, and safe mobility techniques.
- Maintain accurate evaluations, treatment notes, progress reports, and discharge documentation in the Electronic Medical Record (EMR).
- Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, case managers, and other healthcare professionals.
- Participate in interdisciplinary care planning and discharge planning.
- Ensure compliance with infection prevention protocols, HIPAA regulations, patient safety standards, and facility policies.
- Participate in quality improvement initiatives and continuing education activities.
Required Qualifications:
- Doctor of Physical Therapy (DPT), Master's, or Bachelor's degree in Physical Therapy from an accredited program.
- Active Physical Therapist (PT) license in the state of practice.
- Current BLS certification required.
- Minimum 1 year of recent Physical Therapy experience preferred.
- Strong knowledge of musculoskeletal, neurological, orthopedic, cardiopulmonary, and rehabilitation therapy principles.
- Excellent communication, critical-thinking, organizational, and interpersonal skills.
- Experience with Electronic Medical Record (EMR) systems preferred.
Preferred Qualifications:
- Experience in acute care, outpatient, inpatient rehabilitation, skilled nursing facilities (SNF), home health, or long-term care settings.
- Experience treating orthopedic, neurological, post-surgical, geriatric, and medically complex patients.
- Board Certification or specialty certification (Orthopedic, Neurologic, Geriatric, Sports, or Cardiovascular & Pulmonary) preferred.
- Ability to work independently while collaborating effectively with an interdisciplinary healthcare team.

Pay Rate
$68.09 - $72.97/hour
