Position Summary:
We are seeking a dedicated and compassionate Physical Therapist (PT) to evaluate, develop, and implement individualized treatment plans for patients recovering from injury, surgery, illness, or chronic conditions. The Physical Therapist will help restore mobility, reduce pain, improve functional independence, and enhance overall quality of life through evidence-based rehabilitation interventions.
Key Responsibilities:
- Perform comprehensive physical therapy evaluations and functional assessments.
- Develop and implement individualized treatment plans based on patient needs and rehabilitation goals.
- Provide therapeutic exercises, manual therapy, gait training, balance training, neuromuscular re-education, and mobility interventions.
- Evaluate and treat patients with orthopedic, neurological, cardiopulmonary, post-surgical, and other medical conditions.
- Monitor patient progress and modify treatment plans based on clinical outcomes.
- Educate patients and caregivers on home exercise programs, injury prevention, body mechanics, fall prevention, and safe mobility techniques.
- Recommend and train patients in the use of assistive devices, orthotics, prosthetics, and adaptive equipment.
- Maintain accurate evaluations, treatment notes, progress reports, and discharge documentation in the Electronic Medical Record (EMR).
- Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, case managers, and other healthcare professionals.
- Participate in discharge planning and coordinate continuity of care.
- Ensure compliance with infection prevention protocols, HIPAA regulations, patient safety standards, and facility policies.
- Participate in quality improvement initiatives and continuing education activities.
Required Qualifications:
- Doctor of Physical Therapy (DPT) or Master's degree in Physical Therapy from an accredited program.
- Active Physical Therapist (PT) license in the state of practice.
- Current BLS certification required.
- Minimum 1 year of recent Physical Therapy experience preferred.
- Strong knowledge of rehabilitation principles, therapeutic exercise, manual therapy, patient mobility, and functional restoration.
- Excellent communication, critical-thinking, organizational, and interpersonal skills.
- Experience with Electronic Medical Record (EMR) systems preferred.
Pay Rate
$68.09 - $72.97/hour
