Position Summary:
We are seeking a compassionate and skilled Physical Therapist (PT) to evaluate, develop, and implement individualized treatment plans that help patients improve mobility, reduce pain, restore function, and achieve the highest possible level of independence. The Physical Therapist will collaborate with the interdisciplinary healthcare team to provide high-quality, patient-centered rehabilitation services.
Key Responsibilities:
- Evaluate patients' physical function, mobility, strength, balance, range of motion, and rehabilitation needs
- Develop and implement individualized physical therapy treatment plans
- Provide therapeutic exercises, gait training, balance training, manual therapy, and pain management interventions
- Train patients in the use of assistive devices and adaptive equipment
- Monitor patient progress and modify treatment plans based on clinical outcomes
- Educate patients and caregivers on home exercise programs, injury prevention, and safe mobility techniques
- Document evaluations, treatment sessions, progress notes, and discharge summaries in the Electronic Medical Record (EMR)
- Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, case managers, and other healthcare professionals
- Ensure compliance with infection prevention protocols, HIPAA regulations, patient safety standards, and facility policies
- Participate in quality improvement initiatives and continuing education activities
Required Qualifications:
- Doctor of Physical Therapy (DPT) or Master's degree in Physical Therapy from an accredited program
- Active, unrestricted Physical Therapist (PT) license in the state of practice
- Current BLS certification required
- Minimum 1 year of recent physical therapy experience preferred
- Strong knowledge of rehabilitation techniques, therapeutic exercise, patient assessment, and treatment planning
- Excellent communication, organizational, and interpersonal skills
- Experience with Electronic Medical Record (EMR) systems preferred
Pay Rate
$66.37 - $71.24/hour
