Position Summary:
We are seeking a compassionate and skilled Physical Therapist (PT) to provide comprehensive rehabilitation services to patients with injuries, illnesses, disabilities, or functional impairments. The Physical Therapist will evaluate patients, develop individualized treatment plans, implement evidence-based therapeutic interventions, and collaborate with interdisciplinary healthcare teams to restore function, improve mobility, reduce pain, and maximize patient independence and quality of life.
Key Responsibilities:
• Perform comprehensive physical therapy evaluations and functional assessments to determine patient needs and rehabilitation goals
• Develop, implement, and modify individualized treatment plans based on patient diagnoses, functional limitations, and clinical progress
• Provide evidence-based therapeutic interventions, including therapeutic exercise, manual therapy, gait training, balance training, neuromuscular re-education, and functional mobility training
• Evaluate and treat patients with orthopedic, neurological, musculoskeletal, cardiopulmonary, and post-operative conditions
• Monitor patient progress and adjust treatment plans to optimize functional outcomes and recovery
• Educate patients and caregivers on home exercise programs, injury prevention, body mechanics, adaptive strategies, and wellness initiatives
• Recommend and train patients in the use of assistive devices, mobility aids, and adaptive equipment as appropriate
• Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, case managers, and interdisciplinary healthcare teams to coordinate patient care
• Maintain accurate and timely documentation of evaluations, treatment sessions, progress notes, and discharge summaries in electronic medical records (EMR)
• Participate in interdisciplinary care planning, discharge planning, and patient education initiatives
• Ensure compliance with infection prevention protocols, patient safety standards, and regulatory requirements
• Promote patient independence, functional recovery, and improved quality of life through individualized rehabilitation programs
Required Qualifications:
• Active, unrestricted Physical Therapist (PT) license in the state of practice
• Graduate of an accredited Physical Therapy program
• Doctor of Physical Therapy (DPT) degree required (or equivalent educational qualifications as accepted by state licensing requirements)
• Current BLS/CPR certification required
• Minimum 1–2 years of recent Physical Therapist experience preferred
• Strong knowledge of physical therapy principles, anatomy, physiology, rehabilitation techniques, and evidence-based practice
• Excellent clinical assessment, treatment planning, and documentation skills
• Strong communication, organizational, and interpersonal abilities
• Ability to work independently and collaboratively within a multidisciplinary healthcare team
• Experience with electronic medical record (EMR) systems preferred
• Commitment to providing compassionate, patient-centered, and evidence-based care
Pay Rate
$56.48 - $61.36/hour
