Position Summary:
We are seeking a skilled and compassionate Physical Therapist (PT) to provide professional therapy services to patients across various age groups and healthcare settings. The Physical Therapist will evaluate patient needs, develop and implement individualized treatment plans, provide evidence-based therapeutic interventions, and collaborate with interdisciplinary teams to improve mobility, restore function, enhance independence, and improve overall quality of life.
Key Responsibilities:
• Provide skilled physical therapy services to patients of all ages based on individual needs and functional goals
• Perform comprehensive patient evaluations and develop appropriate treatment plans based on clinical findings
• Plan, implement, and manage individualized physical therapy care plans
• Provide therapeutic interventions including exercises, mobility training, gait training, balance activities, and functional rehabilitation techniques
• Reassess patient progress and modify treatment plans as needed to achieve optimal outcomes
• Apply evidence-based physical therapy principles and current clinical practices to patient care
• Educate patients and caregivers on treatment plans, exercises, safety techniques, and strategies to improve functional outcomes
• Collaborate effectively with physicians, nurses, therapists, patients, families, and healthcare teams
• Maintain accurate and timely documentation of evaluations, treatments, progress notes, and discharge summaries
• Utilize problem-solving and quality improvement strategies to enhance patient care processes
• Participate in discharge planning and provide recommendations for continued care
• Orient, train, and supervise Physical Therapist Assistants (PTAs) according to state guidelines and organizational policies
• Follow patient safety, infection prevention, and regulatory standards
Required Qualifications:
• Active, unrestricted Physical Therapist (PT) license in the state of practice
• Graduate of an accredited Physical Therapy program
• Doctor of Physical Therapy (DPT) degree or equivalent required
• Minimum 1 years of healthcare experience performing similar duties preferred
• Strong clinical assessment, treatment planning, and documentation skills
• Knowledge of current physical therapy practices and patient care standards
• Excellent communication and interpersonal skills
• Ability to work collaboratively within a multidisciplinary healthcare team
• Strong organizational skills and ability to manage multiple patient care priorities
• Commitment to providing compassionate, evidence-based patient care
Pay Rate
$66.37 - $71.24/hour
