Position Summary:

We are seeking an experienced and compassionate Physical Therapist (PT) to provide skilled therapy services to patients in a home health setting. The Physical Therapist will evaluate patient functional status, develop and implement individualized treatment plans, monitor patient progress, educate patients and caregivers, and collaborate with physicians and interdisciplinary teams to promote recovery, functional independence, and safe discharge.

Key Responsibilities:

• Provide skilled physical therapy services in accordance with physician orders and established plans of care
• Conduct comprehensive patient evaluations and functional assessments in the home setting
• Develop, implement, and revise individualized physical therapy treatment plans
• Treat patients to relieve pain, restore function, improve mobility, and maximize independence
• Monitor patient progress toward established goals and modify treatment plans as needed
• Observe, document, and report patient responses to therapy and changes in condition
• Educate patients and caregivers on therapeutic exercises, safety techniques, and use of assistive devices
• Prepare patients and caregivers for discharge when appropriate
• Maintain accurate, timely, and compliant clinical documentation
• Participate in interdisciplinary communication to ensure continuity and coordination of care
• Support quality improvement initiatives and adhere to clinical best practices

Required Qualifications:

• Active, unrestricted Physical Therapist (PT) license in the practicing state
• Bachelor’s degree in Physical Therapy required; Master’s or Doctorate preferred
• Must have 3 years of experience as a Physical Therapist preferred
• Home health experience strongly preferred
• Strong assessment, treatment planning, and clinical decision-making skills
• Excellent communication, documentation, and patient education abilities
• Ability to work independently in a home-based care environment
