Position Summary:

We are seeking a compassionate and skilled Physical Therapist to provide rehabilitation services in a Skilled Nursing Facility (SNF). The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide evidence-based interventions to improve mobility, restore function, reduce pain, and enhance independence while promoting the highest quality of life.


Key Responsibilities:

  • Perform comprehensive physical therapy evaluations and functional assessments.
  • Develop and implement individualized treatment plans based on physician referrals and patient goals.
  • Provide therapeutic exercises, gait training, balance training, manual therapy, transfer training, and mobility interventions.
  • Assess and recommend assistive devices, adaptive equipment, orthotics, and mobility aids.
  • Monitor patient progress and modify treatment plans as needed to achieve rehabilitation goals.
  • Educate patients, families, and caregivers on home exercise programs, fall prevention, body mechanics, and safe mobility techniques.
  • Maintain accurate treatment documentation, progress notes, and discharge summaries in the Electronic Medical Record (EMR).
  • Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other members of the interdisciplinary care team.
  • Participate in care planning meetings and discharge planning to ensure continuity of care.
  • Ensure compliance with infection prevention protocols, HIPAA regulations, patient safety standards, and facility policies.
  • Participate in quality improvement initiatives and continuing education activities.

Required Qualifications:

  • Doctor of Physical Therapy (DPT) or equivalent degree from an accredited Physical Therapy program.
  • Active Physical Therapist (PT) license in the state of practice.
  • Current BLS certification required.
  • Minimum 1 year of recent Physical Therapy experience, preferably in a Skilled Nursing Facility (SNF) or rehabilitation setting.
  • Strong knowledge of geriatric rehabilitation, therapeutic exercise, mobility training, balance training, and functional restoration.
  • Excellent communication, critical-thinking, organizational, and interpersonal skills.
  • Experience with Electronic Medical Record (EMR) systems preferred.

Pay Rate
$68.09 - $72.97/hour
