Position Summary:
We are seeking a compassionate and skilled Physical Therapist to provide rehabilitation services in a Skilled Nursing Facility (SNF). The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide evidence-based interventions to improve mobility, restore function, reduce pain, and enhance independence while promoting the highest quality of life.
Key Responsibilities:
- Perform comprehensive physical therapy evaluations and functional assessments.
- Develop and implement individualized treatment plans based on physician referrals and patient goals.
- Provide therapeutic exercises, gait training, balance training, manual therapy, transfer training, and mobility interventions.
- Assess and recommend assistive devices, adaptive equipment, orthotics, and mobility aids.
- Monitor patient progress and modify treatment plans as needed to achieve rehabilitation goals.
- Educate patients, families, and caregivers on home exercise programs, fall prevention, body mechanics, and safe mobility techniques.
- Maintain accurate treatment documentation, progress notes, and discharge summaries in the Electronic Medical Record (EMR).
- Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other members of the interdisciplinary care team.
- Participate in care planning meetings and discharge planning to ensure continuity of care.
- Ensure compliance with infection prevention protocols, HIPAA regulations, patient safety standards, and facility policies.
- Participate in quality improvement initiatives and continuing education activities.
Required Qualifications:
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited Physical Therapy program.
- Active Physical Therapist (PT) license in the state of practice.
- Current BLS certification required.
- Minimum 1 year of recent Physical Therapy experience, preferably in a Skilled Nursing Facility (SNF) or rehabilitation setting.
- Strong knowledge of geriatric rehabilitation, therapeutic exercise, mobility training, balance training, and functional restoration.
- Excellent communication, critical-thinking, organizational, and interpersonal skills.
- Experience with Electronic Medical Record (EMR) systems preferred.
Pay Rate
$68.09 - $72.97/hour
