Physical Therapist (PT) – Home Health

We are seeking a skilled and compassionate Physical Therapist to provide individualized rehabilitation services to patients in their homes. The Physical Therapist will evaluate functional needs, develop treatment plans, and help patients improve mobility, strength, balance, and independence in a home-based setting.

Responsibilities:

Perform physical therapy evaluations and functional assessments in patients’ homes.
Develop individualized treatment plans based on patient needs and goals.
Provide therapeutic exercises, stretching, manual therapy, and mobility training.
Perform gait, balance, transfer, and strength training.
Assess the home environment and recommend appropriate safety modifications.
Educate patients and caregivers on exercises, home programs, and safe mobility.
Monitor patient progress and modify treatment plans as needed.
Coordinate care with physicians, nurses, and other healthcare professionals.
Maintain accurate patient records and treatment documentation.
Follow infection prevention, patient safety, and professional standards.

Qualifications:

Current Physical Therapist license as required.
Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program.
Previous home health or clinical experience preferred.
Knowledge of rehabilitation, therapeutic exercise, mobility, and home safety.
Strong clinical judgment, assessment, and problem-solving skills.
Excellent communication and interpersonal skills.
Ability to work independently and manage a home-based caseload.
Compassionate, dependable, professional, and patient-focused.

Pay Rate
$79.52 - $84.40/hour
