Position Summary:

We are seeking a compassionate and skilled Physical Therapist (PT) – Home Health to provide individualized therapy services to patients in their homes. The Physical Therapist will evaluate functional mobility, develop personalized treatment plans, and help patients improve strength, balance, mobility, and independence while promoting safe recovery in the home environment.


Key Responsibilities:

  • Evaluate patients' physical function, mobility, strength, balance, and rehabilitation needs in the home setting
  • Develop and implement individualized physical therapy treatment plans
  • Provide therapeutic exercises, gait training, balance training, transfer training, and pain management interventions
  • Assess home safety and recommend adaptive equipment or environmental modifications
  • Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques
  • Monitor patient progress and modify treatment plans as needed
  • Coordinate care with physicians, nurses, occupational therapists, speech-language pathologists, case managers, and other healthcare professionals
  • Maintain accurate and timely documentation in the Electronic Medical Record (EMR)
  • Ensure compliance with home health regulations, infection prevention protocols, HIPAA, and patient safety standards
  • Participate in interdisciplinary case conferences, quality improvement initiatives, and continuing education activities

Required Qualifications:

  • Doctor of Physical Therapy (DPT) or Master's degree in Physical Therapy from an accredited program
  • Active, unrestricted Physical Therapist (PT) license in the state of practice
  • Current BLS certification required
  • Minimum 1 year of recent physical therapy experience, preferably in home health, rehabilitation, or acute care
  • Valid driver's license, reliable transportation, and current auto insurance as required
  • Strong knowledge of rehabilitation techniques, patient assessment, mobility training, and home safety evaluations
  • Excellent communication, organizational, and interpersonal skills
  • Experience with Electronic Medical Record (EMR) systems preferred

Pay Rate
$71.90 - $76.78/hour
