Physical Therapist – Home Health
Position Summary
Provide skilled physical therapy services to patients in their homes to improve mobility, strength, balance, and functional independence. Develop individualized treatment plans and support safe recovery within the home environment.
Responsibilities
- Evaluate patients and develop individualized home health treatment plans.
- Provide therapeutic exercises, gait training, mobility training, and functional activities.
- Assess and address balance, strength, coordination, and range-of-motion limitations.
- Monitor patient progress and modify treatment plans as needed.
- Educate patients and caregivers on exercises, safety, fall prevention, and home programs.
- Recommend appropriate assistive devices and home modifications as needed.
- Maintain accurate and timely clinical documentation.
- Coordinate care with physicians, nurses, and other healthcare professionals.
- Follow home health, patient safety, infection-control, and clinical standards.
Qualifications
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program.
- Current and active Physical Therapist license in the applicable state.
- Previous home health or community-based experience preferred.
- Strong clinical assessment, communication, organizational, and patient-care skills.
- Ability to work independently and manage multiple patient visits effectively.
Pay Rate
$71.90 - $76.78/hour
