Position Summary:
We are seeking a skilled Physical Therapist to provide individualized rehabilitation services to patients in their homes. The PT will evaluate functional abilities, develop treatment plans, provide therapeutic interventions, and educate patients and caregivers to improve mobility, safety, and independence.
Key Responsibilities:
- Conduct comprehensive physical therapy evaluations in the home setting.
- Develop and implement individualized treatment plans based on patient needs and goals.
- Provide therapeutic exercises, gait training, balance training, strengthening, and functional mobility interventions.
- Assess home environments and recommend appropriate safety modifications and assistive devices.
- Monitor patient progress and modify treatment plans as needed.
- Educate patients and caregivers on home exercise programs, fall prevention, mobility, and safe transfers.
- Coordinate care with physicians, nurses, occupational therapists, speech-language pathologists, and other healthcare professionals.
- Communicate changes in patient condition and treatment progress to the appropriate care team.
- Maintain accurate and timely clinical documentation, evaluations, progress notes, and discharge summaries.
- Participate in care planning and interdisciplinary case conferences.
- Follow home health, infection prevention, HIPAA, patient safety, and organizational policies.
Required Qualifications:
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program.
- Active Physical Therapist license in the state of practice.
- Current BLS certification required.
- Minimum 1 year of recent Physical Therapy experience preferred.
- Previous home health experience preferred.
- Strong knowledge of rehabilitation, therapeutic exercise, mobility, and fall prevention.
- Excellent clinical judgment, communication, organization, and time-management skills.
- Ability to work independently and manage multiple home visits.
- Valid driver's license and reliable transportation for patient visits.
Preferred Qualifications:
- Previous Medicare-certified home health experience.
- OASIS experience preferred.
- Experience treating geriatric, orthopedic, neurological, post-surgical, and medically complex patients.
- Strong knowledge of home safety assessments and adaptive equipment.
- Experience coordinating care within an interdisciplinary home health team.
- Commitment to compassionate, patient-centered care.
Pay Rate
$60.47 - $65.35/hour
