Physical Therapist – Senior Living Community

Position Summary:

We are seeking a compassionate and skilled Physical Therapist (PT) to provide rehabilitative and wellness-focused therapy services to residents within a senior living community. The Physical Therapist will evaluate functional mobility, develop individualized treatment plans, and deliver therapy interventions that promote independence, safety, and quality of life for older adults.

Key Responsibilities:

• Conduct comprehensive physical therapy evaluations and functional assessments
• Develop and implement individualized treatment plans focused on mobility, balance, strength, and fall prevention
• Provide therapeutic exercises, gait training, transfer training, and pain management interventions
• Monitor resident progress and adjust treatment plans as clinically indicated
• Educate residents and caregivers on home exercise programs, safe mobility, and fall prevention strategies
• Collaborate with interdisciplinary care teams including nursing, occupational therapy, and wellness staff
• Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and timely
• Participate in wellness, fitness, and prevention programs within the senior living community
• Ensure compliance with clinical standards, safety regulations, and documentation requirements
• Promote a positive, supportive, and resident-centered therapy environment

Required Qualifications:

• Doctorate or Master’s degree in Physical Therapy from an accredited program
• Active, unrestricted Physical Therapist license in the applicable state
• Experience working with geriatric or senior populations preferred
• Strong knowledge of functional mobility, balance training, and fall prevention
• Excellent communication, interpersonal, and patient education skills
• Ability to work independently and collaboratively within a multidisciplinary team
• Commitment to delivering compassionate, resident-centered care

Pay Rate
$52.85 - $57.72/hour
