Position Summary:
We are seeking a skilled Physical Therapist to provide outpatient rehabilitation services to adult patients with orthopedic, musculoskeletal, neurological, and functional conditions. The PT will evaluate patients, develop individualized treatment plans, provide therapeutic interventions, and support patients in achieving improved mobility and independence.
Key Responsibilities:
- Perform comprehensive physical therapy evaluations and functional assessments.
- Develop individualized treatment plans based on patient conditions, goals, and functional needs.
- Provide therapeutic exercises, manual therapy, neuromuscular re-education, gait training, and functional mobility training.
- Treat orthopedic, musculoskeletal, post-operative, neurological, and chronic conditions.
- Monitor patient progress and modify treatment plans as appropriate.
- Educate patients on home exercise programs, injury prevention, body mechanics, and self-management.
- Recommend and train patients in the use of appropriate assistive devices.
- Maintain accurate treatment notes, progress reports, and discharge documentation.
- Communicate effectively with referring providers and other healthcare professionals.
- Participate in care planning, quality improvement, and continuing education activities.
- Follow infection prevention, HIPAA, patient safety, and organizational policies.
Required Qualifications:
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program.
- Active Physical Therapist (PT) license in the state of practice.
- Current BLS certification required.
- Minimum 1 year of recent outpatient physical therapy experience preferred.
- Strong knowledge of orthopedic, musculoskeletal, neurological, and rehabilitation principles.
- Excellent clinical judgment, communication, organization, and interpersonal skills.
- Experience with EMR systems preferred.
Preferred Qualifications:
- Experience treating adult orthopedic and post-surgical patients.
- Experience with sports injuries, chronic pain, balance disorders, or neurological conditions.
- Orthopedic or other specialty certification preferred.
- Strong patient education and exercise prescription skills.
- Ability to manage multiple patients efficiently in an outpatient environment.
- Commitment to compassionate, patient-centered care.
Pay Rate
$83.33 - $88.21/hour
