Position Summary:

We are seeking a compassionate Medical Social Worker to provide psychosocial support and care coordination for hospice and medically complex patients in an outpatient setting. The Social Worker will assess patient and family needs, provide counseling and resources, assist with care planning, and support patients and caregivers throughout the course of care.

Key Responsibilities:

  • Conduct comprehensive psychosocial assessments of patients and families.
  • Identify emotional, social, financial, and practical needs affecting patient care.
  • Provide counseling, emotional support, crisis intervention, and supportive services.
  • Assist patients and families with coping strategies, grief, loss, and end-of-life concerns.
  • Develop and implement individualized psychosocial care plans.
  • Coordinate community resources, financial assistance, transportation, housing, and other support services.
  • Assist patients and families with advance care planning and healthcare decision-making resources.
  • Collaborate with physicians, nurses, hospice staff, therapists, chaplains, and other healthcare professionals.
  • Participate in interdisciplinary care planning and case conferences.
  • Advocate for patient preferences, dignity, safety, and quality of life.
  • Provide education and support regarding hospice services, available resources, and care options.
  • Maintain accurate and timely documentation in the EMR.
  • Follow HIPAA, patient safety, ethical standards, and organizational policies.

Required Qualifications:

  • Master’s degree in Social Work (MSW) from an accredited program preferred; BSW may be accepted based on position requirements.
  • Active social work license in the state of practice, as applicable.
  • Previous healthcare, hospice, palliative care, medical social work, or case management experience preferred.
  • Strong knowledge of psychosocial assessment, counseling, care coordination, and community resources.
  • Excellent communication, interpersonal, organizational, and problem-solving skills.
  • Ability to work compassionately with patients and families experiencing serious illness or end-of-life situations.
  • Experience with EMR systems preferred.

Preferred Qualifications:

  • Previous hospice or palliative care social work experience.
  • LCSW certification preferred.
  • Experience with grief and bereavement counseling.
  • Knowledge of Medicare, Medicaid, hospice services, and community resources.
  • Strong patient advocacy and interdisciplinary collaboration skills.
  • Experience supporting patients with chronic, terminal, or medically complex conditions.
  • Commitment to compassionate, patient-centered care.

Pay Rate
$48.27 - $53.15/hour
