Position Summary:
We are seeking a compassionate and skilled Occupational Therapist (OT) – Nursing & Rehabilitation to provide comprehensive occupational therapy services for patients recovering from illness, injury, surgery, or chronic conditions. The Occupational Therapist will evaluate functional abilities, develop individualized treatment plans, and help patients regain independence in activities of daily living (ADLs) while collaborating with the interdisciplinary rehabilitation team.
Key Responsibilities:
- Evaluate patients' physical, cognitive, sensory, and functional abilities
- Develop and implement individualized occupational therapy treatment plans
- Provide therapeutic interventions to improve activities of daily living (ADLs), fine motor skills, upper extremity function, coordination, balance, and mobility
- Train patients in the use of adaptive equipment, assistive devices, and compensatory techniques
- Monitor patient progress and modify treatment plans as needed
- Educate patients, families, and caregivers on home exercise programs, safety techniques, and functional independence
- Complete accurate evaluations, treatment notes, progress reports, and discharge documentation in the Electronic Medical Record (EMR)
- Collaborate with physicians, nurses, physical therapists, speech-language pathologists, case managers, and other healthcare professionals
- Participate in interdisciplinary care conferences and discharge planning
- Ensure compliance with infection prevention protocols, HIPAA regulations, patient safety standards, and facility policies
- Participate in quality improvement initiatives and continuing education activities
Required Qualifications:
- Master's or Doctoral degree in Occupational Therapy from an accredited program
- Active, unrestricted Occupational Therapist (OT) license in the state of practice
- Current NBCOT certification required where applicable
- Current BLS certification required
- Minimum 1 year of recent occupational therapy experience in a rehabilitation, skilled nursing, or post-acute care setting preferred
- Strong knowledge of rehabilitation principles, patient assessment, adaptive equipment, and treatment planning
- Excellent communication, organizational, and interpersonal skills
- Experience with Electronic Medical Record (EMR) systems preferred
Pay Rate
$52.85 - $57.72/hour
