Job Title: Pharmacist

Job Summary:
We are seeking an experienced Pharmacist to provide comprehensive pharmacy and medication management services in both inpatient and outpatient settings. The Pharmacist will ensure safe, effective, and compliant medication use while delivering high-quality patient care across all age groups, including neonatal, pediatric, adolescent, adult, geriatric, oncology, and obstetric populations. This role requires collaboration with healthcare providers to optimize patient outcomes and maintain pharmacy operations in accordance with organizational policies and professional standards.

Key Responsibilities:

  • Provide pharmaceutical care and medication management services to patients.
  • Review, verify, and dispense medications accurately and safely.
  • Counsel patients and healthcare staff on medication therapies and drug interactions.
  • Monitor patient medication regimens and recommend appropriate interventions.
  • Maintain compliance with pharmacy regulations, policies, and quality standards.
  • Collaborate with physicians, nurses, and interdisciplinary healthcare teams.
  • Participate in clinical pharmacy services for inpatient and outpatient departments.
  • Support medication safety initiatives and continuous quality improvement activities.
  • Maintain accurate documentation within the electronic medical record system.
  • Participate in rotational coverage, including weekends, holidays, and other assigned shifts as needed.

Requirements:

  • Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy degree from an accredited pharmacy program.
  • Current, active, and unrestricted Pharmacist license in any U.S. state or territory.
  • Current BLS (Basic Life Support) certification.
  • Minimum of 2 years of experience as a Registered Pharmacist.
  • Minimum of 1 year of clinical pharmacy experience.
  • Strong knowledge of pharmacy operations, medication management, and patient care practices.
  • Experience using electronic health record systems preferred.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Holiday coverage required.

Experience Required:

  • Minimum 3 years of relevant pharmacy experience.

EMR System:

  • Allscripts

Certifications Required:

  • Active Pharmacist License
  • BLS (Basic Life Support)

Pay Rate
$75.71 - $80.59/hour
