Position Summary:

We are seeking a compassionate and dedicated Occupational Therapist (OT) to provide rehabilitation services in a Skilled Nursing Facility (SNF). The Occupational Therapist will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve functional independence, enhance activities of daily living (ADLs), and maximize quality of life.


Key Responsibilities:

  • Perform comprehensive occupational therapy evaluations and functional assessments.
  • Develop and implement individualized treatment plans based on physician referrals and patient goals.
  • Provide therapeutic interventions to improve self-care, mobility, strength, coordination, cognition, and upper extremity function.
  • Train patients in activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
  • Recommend and educate patients on adaptive equipment, assistive devices, splints, and environmental modifications.
  • Monitor patient progress and modify treatment plans to achieve rehabilitation goals.
  • Educate patients, families, and caregivers on home exercise programs, safety techniques, fall prevention, and adaptive strategies.
  • Maintain accurate evaluations, treatment notes, progress reports, and discharge documentation in the Electronic Medical Record (EMR).
  • Collaborate with physicians, nurses, physical therapists, speech-language pathologists, case managers, and other healthcare professionals.
  • Participate in interdisciplinary care planning and discharge planning.
  • Ensure compliance with infection prevention protocols, HIPAA regulations, patient safety standards, and facility policies.
  • Participate in quality improvement initiatives and continuing education activities.

Required Qualifications:

  • Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program.
  • Active Occupational Therapist (OT) license in the state of practice.
  • Current BLS certification required.
  • Minimum 1 year of recent Occupational Therapy experience, preferably in a Skilled Nursing Facility (SNF) or rehabilitation setting.
  • Strong knowledge of geriatric rehabilitation, ADL training, cognitive rehabilitation, adaptive equipment, and functional mobility.
  • Excellent communication, critical-thinking, organizational, and interpersonal skills.
  • Experience with Electronic Medical Record (EMR) systems preferred.

Pay Rate
$64.28 - $69.16/hour
