Occupational Therapist – Home Health
We are seeking a skilled Occupational Therapist – Home Health to provide individualized therapy services in patients’ homes and help them improve functional independence, safety, and quality of life.
Key Responsibilities:
Evaluate patients in their home environment and develop individualized treatment plans.
Provide therapeutic activities to improve mobility, strength, coordination, and daily living skills.
Assess home safety and recommend appropriate adaptive equipment or techniques.
Educate patients and caregivers on exercises, home programs, and safe daily activities.
Monitor patient progress and modify treatment plans as needed.
Maintain accurate clinical documentation and communicate with the healthcare team.
Qualifications:
Graduate of an accredited Occupational Therapy program.
Current OT license in the applicable state.
Current NBCOT certification or equivalent.
Current BLS certification.
Previous home health, outpatient, or rehabilitation experience preferred.
Strong communication, organization, assessment, and patient-care skills.
Pay Rate
$56.66 - $61.53/hour
