Position Summary:

We are seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality nursing care in a Long Term Acute Care (LTAC) or Long Term Care (LTC) setting. The RN will assess, plan, implement, and evaluate patient care for individuals with chronic illnesses, complex medical conditions, and rehabilitation needs while collaborating with an interdisciplinary healthcare team to ensure optimal patient outcomes.


Key Responsibilities:

  • Perform comprehensive patient assessments and develop individualized care plans.
  • Provide direct nursing care, including medication administration, IV therapy, wound care, and treatments.
  • Monitor patient conditions and respond promptly to changes in clinical status.
  • Manage patients with chronic illnesses, ventilators, tracheostomies, feeding tubes, and other complex medical needs as applicable.
  • Coordinate admissions, transfers, and discharges.
  • Educate patients and families regarding medications, treatments, disease management, and discharge planning.
  • Maintain accurate documentation in the Electronic Medical Record (EMR).
  • Collaborate with physicians, therapists, social workers, case managers, and other healthcare professionals.
  • Ensure compliance with infection prevention protocols, HIPAA regulations, patient safety standards, and facility policies.
  • Participate in quality improvement initiatives and support a patient-centered care environment.

Required Qualifications:

  • Active, unrestricted Registered Nurse (RN) license in the state of practice.
  • Graduate of an accredited Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) program.
  • Current BLS certification required.
  • Minimum 1 year of recent LTAC, LTC, Med/Surg, Skilled Nursing, or Acute Care nursing experience preferred.
  • Strong knowledge of chronic disease management, medication administration, wound care, and patient assessment.
  • Excellent communication, critical-thinking, organizational, and interpersonal skills.
  • Experience with Electronic Medical Record (EMR) systems preferred.

Pay Rate
$52.85 - $57.72/hour
