Job Title: Family Nurse Practitioner (FNP)
Position Summary
The Family Nurse Practitioner (FNP) provides comprehensive primary care services to diverse patient populations, including adult and geriatric patients, in a collaborative and patient-centered environment. This role focuses on preventive care, chronic disease management, patient education, and delivering high-quality clinical care to support positive patient outcomes.
Responsibilities
- Provide primary care services including preventive, acute, and chronic care management
- Assess, diagnose, and treat a wide range of medical conditions
- Manage chronic diseases and pain management treatment plans
- Educate patients on wellness, disease prevention, and healthy lifestyle practices
- Collaborate with physicians and multidisciplinary healthcare teams to ensure quality patient care
- Maintain accurate and timely patient documentation and medical records
- Monitor patient progress and adjust treatment plans as needed
Qualifications
- Active Family Nurse Practitioner (FNP) license required
- Active DEA license required
- Previous experience in primary care or related clinical setting preferred
- Strong clinical judgment and patient-centered approach to care
- Excellent communication, interpersonal, and teamwork skills
- Ability to work independently and collaboratively within a healthcare team

- Compensation commensurate with experience
