Position Summary:
We are seeking an experienced Wound Care Nurse Practitioner to provide specialized wound management services to patients residing in long-term care and skilled nursing environments. The NP will assess, diagnose, and treat acute and chronic wounds, develop individualized treatment plans, prescribe appropriate therapies, and collaborate closely with interdisciplinary care teams to promote healing and improve patient outcomes.
Key Responsibilities:
• Provide comprehensive wound care services for patients in long-term care settings
• Assess, diagnose, and manage chronic, acute, and complex wounds
• Treat diabetic, arterial, venous, pressure, mixed-etiology wounds, and Kennedy ulcers
• Stage pressure injuries in accordance with NPIAP guidelines
• Develop and implement individualized wound care treatment plans
• Prescribe medications and wound-related therapies within scope of practice
• Order, review, and interpret diagnostic tests related to wound management
• Utilize advanced wound care products such as foams, alginates, collagens, and silver dressings
• Manage wound vacs and pressure-relieving or reducing devices
• Communicate regularly with nursing staff regarding patient progress and care plan updates
• Collaborate with physicians and interdisciplinary team members
• Document patient assessments, treatments, and outcomes accurately in the EMR
Required Qualifications:
• Active, unrestricted Nurse Practitioner license in the state of Ohio
• Board certification as a Nurse Practitioner
• Master’s degree in Nursing (FNP or related specialization preferred)
• Clinical experience in wound care strongly preferred
• Experience working in long-term care or skilled nursing settings preferred
• Strong knowledge of wound care best practices and advanced treatment modalities
• Proficiency with electronic medical records (EMR) systems
• Excellent communication, clinical judgment, and organizational skills
• Ability to work independently while collaborating effectively within a care team
Pay Rate
$41.46 - $46.34/hour
